The lyrics are from "You and I" by Pvris (great song btw). 

Open this background in PicsArt.

Then add your face claim and before you press the check mark, tap the wand on the top left. Then tap Artistic and Polygon.

Play around with the settings till your satisfied. Then press the check mark on the top right.

Next, tap the same wand and add your black and white filter. Press the check mark.

Before you press the check mark, move the photo where you want it and press overlay. Erase the edges and press the check mark.

Next, add the same photo, tap the wand on the top left, go to artistic, then shear. Play around with the settings. This is what I used. Once you're done, tap the check mark on the top right.

Then press the same wand, and add your black and white filter. Press the check mark.

Press lighten on the drop menu next to the opacity. Then erase the edges.

Save your photo and open in Phonto!
